    Overweight before age 40 increases cancer risk

    In an international study, lead by the University of Bergen, the researchers wanted to find out how adult overweight (BMI over 25) and obesity (BMI over 30) increase the risk of different types of cancer.

    The study showed that if you were overweight before age 40, the risk of developing cancer increases by:

    70 percent for endometrial cancer.
    58 percent for male renal-cell cancer.
    29 percent for male colon cancer.
    15 percent for all obesity-related cancers (both sexes).

    "Obesity is an established risk factor for several cancers. In this study, we have focused on the degree, timing and duration of overweight and obesity in relation to cancer risk," says Professor Tone Bjørge, at Department of Global Public Health and Primary Care, University of Bergen.

    The Effects of Melatonin Supplementation on Parameters of Mental Health, Glycemic Control, Markers of Cardiometabolic Risk, and Oxidative Stress in Diabetic Hemodialysis Patients: A Randomized, Double-Blind, Placebo-Controlled Trial.


    OBJECTIVE:
    This study evaluated the effects of melatonin supplementation on parameters of mental health, glycemic control, markers of cardiometabolic risk, and oxidative stress in diabetic hemodialysis (HD) patients.

    DESIGN:
    A randomized, double-blind, placebo-controlled clinical trial was conducted in 60 diabetic HD patients, 18-80 years of age. Participants were randomly divided into 2 groups to take either melatonin (2 x 5mg/day) (n = 30) or placebo (n = 30) 1 hour before bedtime for 12 weeks. The effects of melatonin on mental health, metabolic status, and gene expression related to metabolic status were assessed using multiple linear regression adjusting for age and BMI.

    RESULTS:
    Melatonin supplementation significantly decreased Pittsburgh Sleep Quality Index (P = .007), Beck Depression Inventory index (P = .001), and Beck Anxiety Inventory index (P = .01) compared with the placebo. Additionally, melatonin administration significantly reduced fasting plasma glucose (β = -21.77 mg/dL, 95% CI -33.22 to -10.33, P < .001), serum insulin levels (β = -1.89 μIU/mL, 95% CI -3.34 to -0.45, P = .01), and homeostasis model of assessment-insulin resistance (β = -1.45, 95% CI -2.10 to -0.80, P < .001), and significantly increased the quantitative insulin sensitivity check index (β = 0.01, 95% CI 0.007-0.02, P < .001) compared with placebo treated subjects. In addition, melatonin administration resulted in a significant reduction in serum high sensitivity C-reactive protein (β = -1.92 mg/L, 95% CI -3.02 to -0.83, P = .001) and plasma malondialdehyde (β = -0.21 μmol/L, 95% CI -0.36 to -0.06, P = .005); also, significant rises in plasma total antioxidant capacity (β = 253.87 mmol/L, 95% CI 189.18-318.56, P < .001) and nitric oxide levels (β = 2.99 μmol/L, 95% CI 0.71-5.28, P = .01) were observed compared with the placebo.

    CONCLUSION:
    Overall, melatonin supplementation for 12 weeks to diabetic HD patients had beneficial effects on mental health, glycemic control, inflammatory markers, and oxidative stress.

    We also assessed the expression of an array of genes involved in metabolism in the liver in response to SR9011 treatment. The expression of lipogenic genes (Srebf1), fatty acid synthase (Fasn), and stearoyl-CoA-desaturase 1 (Scd1) were clearly altered with SR9011 treatment (Fig. 4b). Both Srebf1 and Scd1 expression were suppressed whereas the phase of Fasn was phase shifted (Fig. 4b). Expression of genes involved in cholesterol and bile acid metabolism were also altered. Srebf2 and cholesterol 7α-hydroxylase expression (Cyp7a1) were decreased (Fig. 4b). 3-hydroxy-3-methyl-glutaryl-CoA reductase (Hmgcr) was unaffected in this acute model (Fig. 4b). Peroxisome proliferator-activated receptor gamma coactivator 1-alpha and 1-beta (Ppargc1a and Ppargc1b) both displayed a strong circadian pattern of expression that was suppressed with SR9011 treatment (Fig. 4b). Examination of gene expression in skeletal muscle revealed a potential mechanism for the increased metabolic rate that we observed in the CLAMS experiments. Expression of the genes that encode the rate limiting enzyme for β-oxidation of fatty acids, carnitine palmitoyltransferase 1b (Cpt1b), as well as fatty acid transport into the skeletal muscle, fatty acid transport protein 1 (Fatp1), were elevated (Fig. 4c). Pppargc1b expression was also elevated along with uncoupling protein 3 (Ucp3) consistent with altered fatty acid metabolism in skeletal muscle (Fig. 4c). The pattern of expression followed the expected diurnal increase in expression of genes involved in fatty acid oxidation, but the increases in expression were amplified (Fig. 4c). When we examined the expression key enzymes in the glycolytic pathway (hexokinase (Hk1) and pyruvate kinase (Pkm2)) we noted an increase in both (Fig. 4c) suggesting that treatment resulted increased glucose oxidation in addition to fatty acid oxidation. The rate-limiting enzyme in mammalian nicotinamide adenine dinucleotide (NAD+) biosynthesis, nicotinamide phosphoribosyltransferase (NAMPT), has recently been demonstrated to be expressed in a circadian manner leading to a circadian pattern of NAD+ production and thus regulation of the NAD+-dependent deacetylase SIRT121,22. We found that SR9011 suppressed the circadian rhythm of Nampt gene expression in the liver (Supplementary Fig. 15a) suggesting that REV-ERB agonist treatment may alter post-translational acetylation of proteins that may contribute to some of the physiological alterations that we observe.

    In contrast to the muscle where there was amplification of the circadian expression of genes coupled to fatty acid oxidation and glycolysis, in the white adipose tissue (WAT) we observed a suppression of circadian expression of key genes involved in lipid storage. The expression of both diglyceride acyltransferase 1 and 2 (Dgat1 and Dgat 2), the genes encoding the enzyme that catalyzes the terminal and committed step in triglyceride synthesis, were suppressed with SR9011 treatment (Fig. 4d). Consistent with this pattern, the circadian expression of another gene involved in triglyceride synthesis, monoacylglycerol acyltransfease (Mgat), is also disturbed (Fig. 4d). Expression of lipid droplet associated protein genes including perilipin 1 (Plin1) and hormone sensitive lipase (Hsl) were also suppressed with SR9011 treatment (Fig. 4d). We also observe suppression of Nocturnin (Ccrn4l) expression in both adipose and hepatic tissue (Supplementary Fig. 15b). Similar effects on gene expression were noted in animals treated with SR9009 (data not shown).

    Clearly, modulation of REV-ERB activity by a synthetic agonist alters the pattern of expression of many genes involved in metabolism in several tissues including the liver, skeletal muscle and WAT. The alterations that we observed are consistent with decreased lipogenesis and cholesterol/bile acid synthesis in the liver, increased lipid and glucose oxidation in the skeletal muscle, and decreased triglyceride synthesis and storage in the WAT.

    Peripheral clocks operative in metabolic organs are regulating glucose homeostasis. Circadian control of glucose metabolism implies the combination of central and peripheral regulators. The SCN, being a principal sensor of extrinsic cues, coordinates the function of peripheral oscillators via neuroendocrine signals and feeding behavior. The reciprocal circadian release of hepatokines, myokines, and adipokines by respective metabolic tissues represents a feedback loop of glucose homeostasis, further modulating central and peripheral regulators.

    The Effectiveness of Intermittent Fasting to Reduce Body Mass Index and Glucose Metabolism: A Systematic Review and Meta-Analysis

    Abstract: The effects of an intermittent fasting diet (IFD) in the general population are still controversial. In this study, we aimed to systematically evaluate the effectiveness of an IFD to reduce body mass index and glucose metabolism in the general population without diabetes mellitus. Cochrane, PubMed, and Embase databases were searched to identify randomized controlled trials and controlled clinical trials that compared an IFD with a regular diet or a continuous calorie restriction diet. The effectiveness of an IFD was estimated by the weighted mean difference (WMD) for several variables associated with glucometabolic parameters including body mass index (BMI) and fasting glucose. The pooled mean differences of outcomes were calculated using a random effects model. From 2814 studies identified through a literature search, we finally selected 12 articles (545 participants). Compared with a control diet, an IFD was associated with a significant decline in BMI (WMD, −0.75 kg/m2; 95% CI, −1.44 to −0.06), fasting glucose level (WMD, −4.16 mg/dL; 95% CI, −6.92 to −1.40), and homeostatic model assessment of insulin resistance (WMD, −0.54; 95% CI, −1.05 to −0.03). Fat mass (WMD, −0.98 kg; 95% CI, −2.32 to 0.36) tended to decrease in the IFD group with a significant increase in adiponectin (WMD, 1008.9 ng/mL; 95% CI, 140.5 to 1877.3) and a decrease in leptin (WMD, −0.51 ng/mL; 95% CI, −0.77 to −0.24) levels. An IFD may provide a significant metabolic benefit by improving glycemic control, insulin resistance, and adipokine concentration with a reduction of BMI in adults.

    Sleep debt: the impact of weekday sleep deprivation on cardiovascular health in older women

    Results
    Mean age was 72.1 ± 6.0 years. Compared to women with no sleep debt, women with sleep debt were more likely to be obese and have hypertension (pall < .05). Linear regression models adjusted for age and race/ethnicity revealed that sleep debt was significantly associated with poorer ICH (B = –0.13 [95% CI = –0.18 to –0.08]). The relationship was attenuated but remained significant after adjustment for education, income, depression/anxiety, cumulative stress, and snoring.

    Conclusion
    Sleep debt was associated with poorer ICH, despite taking into account socioeconomic status and psychosocial factors. These results suggest that weekly sleep duration variation, possibly leading to circadian misalignment, may be associated with cardiovascular risk in older women.

    Influence of Obesity, Weight Loss, and Free Fatty Acids on Skeletal Muscle Clock Gene Expression.

    Abstract
    The molecular circadian clock plays a role in metabolic homeostasis. We tested the hypothesis obesity and systemic factors associated with insulin resistance affect skeletal muscle clock gene expression. We determined clock gene expression in skeletal muscle of obese women (n=5) and men (n=18) before and 6 months after Roux-en-Y gastric bypass (RYGB) surgery and normal weight controls (women n=6, men n=8). Skeletal muscle clock gene expression was affected by obesity and weight loss. CRY1 mRNA (P=0.05) was increased and DBP mRNA (P<0.05) was decreased in obese versus normal weight women and restored to control levels after RYGB-induced weight loss. CLOCK, CRY1, CRY2 and DBP mRNA (P<0.05) was decreased in obese men as compared to normal weight men. Expression of all other clock genes was unaltered by obesity or weight loss in both cohorts. We correlated clock gene expression with clinical characteristics of the participants. Among the genes studied, DBP and PER3 expression was inversely correlated with plasma lipids in both cohorts. Circadian time-course studies revealed that core clock genes oscillate over time (P<0.05), with BMAL1, CIART, CRY2, DBP, PER1 and PER3 expression profiles altered by palmitate treatment. In conclusion, skeletal muscle clock gene expression and function is altered by obesity, coincident with changes in plasma lipid levels. Palmitate exposure disrupts clock gene expression in myotubes, indicating dyslipidemia directly alters the circadian program. Strategies to reduce lipid overload and prevent elevations in NEFA and cholesterol levels may sustain circadian clock signals in skeletal muscle.
